Assess cognitive function, including learning, thing, memory, judgement, and language. May be required when there are concerns regarding a child’s cognition, and if they are showing signs of cognitive impairment.

Functional Assessment
Assess a person’s current functional status, identified areas where they may require support or assistance, Can be common for people with NDIS funding to evaluate ability to perform daily tasks at home, work and in the community
Fees and Deposits
Payment of the deposit amount is required at the time of booking the initial appointment.
Please note that these assessments are not eligible for a Medicare rebate.
As a practice we do not invoice 3rd parties, eg NDIS, directly for these assessments. Invoices are available following each session.
Cancellation Policy
Deposit payments are non-refundable if the client cancels the assessment within 7 days of the initial consultation
Failure to attend a subsequent appointment, or if the client cancels within 7 days of the scheduled appointment time, a broken appointment fee of $100 per hour of appointment time will be payable. Payment is required prior to the appointment being able to be rescheduled.
Report Writing
Report will be completed within approx. 6 weeks of final assessment session (all questionnaires will be required to be completed as well) and will be given to client at the final assessment session.
